up to theendof the following calendar year at anyof the above clinics. AQueenslandHealth
consent formmust be completed and signedon theday.
The following vaccinations areoffered FREE:
Year 8 students (all)
Year 9 students
Year 10 students
• Diphtheria/tetanus/pertussis
(whooping cough)
• Varicella - zoster (chickenpox)
• HPV (humanpapillomavirus
vaccine) (3doses)
• Hepatitis B (2doses) available
to studentswhowerenot
vaccinated in2013.
